Hey guys,

Here's our complete, bonus interview with Dr. Martina Cartwright, a dermatologist who is on the frontier of studying the effects of Xylazine (Tranq) on the body. This is my first time interviewing a doctor, so I'm stoked!

Also, I did this because you kind Patrons let me know that my inferences about Necrotizing fasciitis where a little off base, so thanks for keeping the journalism blade sharp!

Appreciate the content! Just wanted to give you a heads up, while the interview was really interesting and Dr Cartwright did a good job explaining some topics, she is not a dermatologist. Just from a quick google search I can see she is a dietician and has a PhD, but this does not qualify her has a medical doctor (MD/DO) that is board certified in dermatology. To get greater detail, information, and insight on the issue, I would look into talking to pain medicine, addiction medicine, and other general physicians dealing with the issue in a true clinical setting.

Sad world, everyone's on drugs. Not judging, I want to help so all don't feel the need to escape with them. Sadly, cliche as it is, every addict has to want to find another stress relief method before discussion of drug removal becomes remotely beneficial. I don't see that happening for those whose habits have lowered them to the streets, nevermind Tranq addiction. Outreach required.

Andrew, I work in pain management. Your interview was spot on for all the topics covered. Dr. Cartwright knows what she is talking about. Especially about infection not being contained to the injection site. Sepsis spreads through the blood because it's basically an inflammatory infection of the blood. Your coverage is accurate and gives a lot of insight for any harm reduction educator. One of the reasons people are getting these infections is that they believe it's only missing a vein that causes it. That's heroin user thinking. Tranq causes this stuff by dint of the drug itself and not by its route of administration. If your viewers want more info than that they can go to med school, lol.

Emergency physician here. This approaches medical accuracy and is an improvement on the initial segment, but again misses the mark. What many people who use xylazine intramuscularly (shooting into muscle on accident or on purpose) or intravenously develop are necrotic ulcers. This is NOT necrotizing fasciitis. However, a necrotic ulcer likely increases the risk of necrotizing fasciitis, but it is a VERY different entity. A necrotic ulcer simply means there is dead tissue. You can have dead tissue without an infection. Necrotizing fasciitis is an infection caused by bacteria that ultimately invade the fascial tissue and can spread very rapidly. Necrotizing fasciitis is only diagnosed by surgery and many of these patients will die in a matter of hours to days despite aggressive surgical resection, heavy duty antibiotics, and ICU care. As other viewers pointed out, Dr. Cartwright is indeed a doctor of philosophy and not a medical doctor. I do not blame her for not having a full understanding of the pathophysiology of necrotizing fasciitis.

I used to perform veterinary surgeries as a laboratory neuroscientist, and we used xylazine in our anesthetic cocktail. You have to be SO careful, especially if you need to do multiple surgeries (so multiple administrations of xylazine per animal). It's tragic but unsurprising to see it gaining popularity on the streets. Ps. The lab coat + sweat pants + comfy shoes is so legit, that's the outfit of a scientist with her priorities straight.

Pharmacologist/medicinal chemist here. Interesting interview. One minor thing to point out - while both ketamine and xylazine are used as anesthetics the mechanism of action (how they work) is very different. Ketamine acts (for the most part) as an antagonist at the NMDA receptor, a type of glutamate ion channel (it acts as a channel blocker). Other NMDAR antagonists include PCP and dextromethorphan (DXM). Xylazine acts predominantly as an agonist at the alpha2 adrenergic receptor (similar to clonidine). This reduces the presynaptic release of norepinephrine and has a sedating/anesthetic effect. I study ketamine for a living so felt obligated to point that out! Also I think what is really "evil" is that the government will not do its job and regulate these markets and offer people evidence based medical services to help reduce harm and get them help. While I understand the sentiment, the notion that banning a substance would have any real impact on availability is absurd, aside from MAYBE shifting the market to other similarly acting drugs - usually less well tested and understood. There is clearly a demand for "unapproved" psychoactive drugs that is going no where and prohibition has been an absolute dumpster fire of failure that exacerbates all the inherent harms and creates many new ones. Frustrating to see the largely avoidable loss of life and harm day after day.

Sure thing. I do work developing new NMDAR antagonists (mostly for pain indications but also very interested in use in depression) as well as studying their pharmacological effects (this includes ketamine) for basic science but also to improve future drug development projects. So med chem and pharmacology. As to your thinking - my take is like most substances, ketamine has many potential uses some of them easy to call therapeutic, others easier to call non-therapeutic, and other harder to place uses which may fall in between. But there is no question ketamine has demonstrated powerful rapid acting antidepressant effects in many randomized controlled clinical trials. Spravato (a nasal spray which contains an enantiomer of ketamine called esketamine) is FDA approved as an add-on for TRD and MDD associated with acute suicidal ideation). There are also smaller but still relevant clinical studies supporting efficacy in suicidal ideation (the thoughts themselves in this case), pain, substance use disorder and more.

just to hop on this bandwagon - there are a ton of other providers with real credentials and on-the-ground experience who you could have spoken with for this interview. The wound care nurses at Prevention Point are a wealth of knowledge on the topic and have been doing a lot of work to propagate best practices in the face of increased xylazine prevalence. Ditto, her mention of people not staying in the hospital or keeping appointments in order to chase highs papers over a HUGE problem in the community, which is provider stigma and inadequate withdrawal management in the in-patient setting. Xylazine and fentanyl withdrawal play a big role there and if you had asked ANYONE on the Ave, they would have told you the same.

Nah no way man, doesn’t matter who’s in power drugs are getting into the country. Addiction is more of a social and economic issue. The problems mainly lay within the disproportion of wealth in your country and the lack of access to a lot of basic needs due to cost and privatisation. In Australia we have a lot of wealth inequality too, not close to you guys but a key factor is the accessibility of services that prevent drug addiction

Because there is a miscommunication between the two. The chronic ulcers that these people suffer with is not necrotizing fasciitis. Nec fasc is an acute, rapidly progressive, deep tissue infection. IV drug users are also at an increased risk for nec fasc, which is where the confusion stems from. But conflating the two conditions is medically incorrect, as they have very different pathophysiology, different treatments, and wildly different prognoses.

The information she gave is correct but "French Homie" is also correct - she is not a Fellow of the American Academy of Dermatology (FAAD) and is not certified by the American Board of Dermatology. MDs, and the government, take false representations of being a medical doctor pretty seriously.

Yeah to say someone can't/shouldn't be fact-checked because they have a degree is ridiculous. American's blind faith in medical professionals is why people will take anything their doctor prescribes without a second thought. That mentality/ignorance is great for big pharma though. She refers to xylazine as being like ketamine and she also says it rivals heroin. Xylazine doesn't affect the same neurotransmitters as dissociative or opioids, at least not in any way close enough to categorize it as a dissociative or opioid. There's absolutely nothing enjoyable about tranq unless you like feeling unbelievably tired and nodding off constantly with no euphoria. The fent adds euphoria to it I guess, but even then, most people would rather be doing pure heroin, oxycodone, oxymorphone, or hydromorphone than fentanyl. Fentanyl is less enjoyable and lasts 1/4 or less as long as H. After the government made it nearly impossible to be prescribed opioids these counterfeit pills pressed with unknown doses of fent started appearing. Addiction, use, and deaths have only increased. So once again the government has made the problem worse. Learning nothing from alcohol prohibition or the decades of this failed drug war.
