In the vision right after the children's death, Oda said he "can't write the story any longer" precisely because he feels that it's his own fault the children were killed. He believed his actions and choices were what led to their deaths. I mean obviously you can assume that he'll start killing people after this but for that specific moment for that vision, it was because of the orphans' death that he felt responsible for. In retrospect, you can also say that moment was when he decided to kill and that vision was just supporting his decision to do so? I guess it can be interpreted in more ways than one but for me personally, I think it's the former of what I stated.
