I'm always wondering . . . at some point in the process I imagine you decide things like "no that arm couldn't possibly go here, I need to move everything around." Does that kind of thing mostly happen in your head before you get to this stage? Or do you realize things like that while sketching?

It's a little of both, sometimes things you visualize in your head don't work as well as you thought they would when fully drawn out, so there are times where things need to be adjusted while drawing.
