I think Sam could fight back than because he that was when he was learning and practicing his skills and it was just a scoop yard fight so there was no guarente that the other kid actually knew how to fight. He was just using his size to his advantage. Dean even commented in the very first season that he became lax in his fighting skills.

Another fun reaction. I've always enjoyed this episode too - I love getting to know the back story of the boys. It gives us so much insight into their characters. (BTW Fandom calls the younger versions of S&D the "Weechesters".) Plus I like the message the episode conveys - that we never know what another person is dealing with, so we should all try to approach each other with understanding and kindness. You asked why young Sam was so much smaller than young Dean - I think it's due to a number of things: 1) they really wanted to use Colin Ford again as Sam, 2) the ages they were each supposed to be in the episode, Dean would have gone through a growth spurt, whereas Sam was still pretty young. (Dean @ 18, Sam @ 14) and 3) Jared has said that he was a "scrawny" kid all the way through high school, so it kind of mirrors his real life.

The boys were using salt rounds in the shot guns, so they didn't kill the driver. Salt rounds apparently sting like hell, but they don't go deeper than skin or a layer of fat. Not enough penetrating power to hurt any organs. Sam and Dean just drove Dirk's ghost out of the driver's body.
