I personally think 2 or 3 should be picked of the choices. #4 has the downside of not really being intuitive on what is equipped where and what if any limitations are there. 2 and 3 gives a more clear indication that only certain equipment can be wore in certain slots and indicates how many of said item can be equipped. For instance #4 kinda gives the player a vague idea that they can wear multiple helmets as no visual limitation is given. However in 2 and 3 there is clearly only 1 slot available near where a helmet would go.
