There are several reasons you can’t have an auto kill switch. Not everyone on Tartarus is bad, some people were put there because their quirk is too dangerous to society, so flooding the chambers would kill innocents. Another reason is, there are a lot of people there with all kinds of quirks, and there is a high possibility there are water based quirks. What if flooding the place with water made them stronger? It would be hard to come up with a kill method that would work on everyone. Also, it would t probably cause an uproar. There are people that already see Taurtaros as inhumane, now imagine if you implemented an auto kill switch and killed everyone? Hero society would crumble even more than it already is. Now, with that being said, they should have used the guns in All For One’s cell as soon as they were notified about the nomu’s. If they would have killed him before the system went down then this wouldn’t have happened.

Ok, the whole flooding the place idea, did it ever occur to u that water would help one of those villains in Tartarus, also there was no way that the guards would ever think that AFO would be able to sync up two bodies with multiple quirks and break everyone out that is way out of the box thinking.

TBF they dropped the ball by leaving AFO alive at all back when All Might first defeated him. His quirks are too dangerous and he is insurmountably malicious. His mere absence would have destroyed his plans
