Alluka and Killua consider Something(Nanika) as another sibling. So to Killua has 2 sisters in that one body. Taking care of her is like when a sibling becomes the guardian of another sibling with a severe disability.....just the Hunter world version of that. Morel told Leorio not to tell Gon about Killua because originally Killua was going to tell Nanika to never return and disappear with Alluka.
