Request Stream FORM (Patreon)

Here's the form for this month's request stream, which I'll be running at 8PM UK time on the 18th. This is the regular sketch requests like I do every month, but if you'd like a Halloween or autumnal theme, please put whatever you  have in mind into the 'personality' box :) Just a reminder that you don't need to be present at the stream in order to make a request, you just need to be a patron.
