They were spoofing on shojo anime/manga so hard there. And everyone had loveydovey shit on the brain, so they were slightly out of character in some ways XD. But I don't think there's an actual love triangle going on. They just did it to poke fun at the shojo genre.

Shojo is a demographic - young/teenage girls (people around Kei's age). Shonen is targeted at young/teenage boys. Then it's josei/seinen for older girls/boys which are meant to be somewhat more "mature" etc. That's why the narrator said it's embarrassing - not the romance, but the fact that they're reading a manga for a completely different demographic. Of course it's a pretty vague concept. For example, Attack on Titan is a shonen show, but Season 3 of the show definitely went into territory that would normally be considered seinen.

That was pretty confusing but if you look at Ishigami's dialogue there's nothing weird about it, he wants to go to the aquarium with Kaguya to thank her for helping him study and to take her to the nurse bc he thought she was sick but didn't want to bother Shirogane, but with the whole shoujo mood everything looked all shiny and exaggerated
