Can she fusion paste another form onto a fusion form, producing a form from three people? And if so, does the ratio end up like 1/2 last form, 1/4 second to last, 1/4 first, or does magic smooth out the ratios for one version of convenience?

A new Fusion Paste would replace the first. She could potentially combine it with some other transformations, especially if the transformed is the one she's scanning, but she can't combine it with itself.

What if she scans a fused person? In that scenario the scan target is already transformed.

Think of it this way, the fusion is an enchantment, but to cast the fusion spell she copies/reads/unzips, the magical "data"/"code" for the default form of the targets of the spell. enchantments don't stack perfectly :D
