The whole thing about Rias asking where she stands is because it's obvious but she loves issei and he loves her but he doesn't know that she loves him and just sees her as his 'president/master' and he calls her president all the time unlike the rest of the characters he calls by name.

I like they said Akeno and Michael tempered with the Sword so that Issei could use it. And aside from that Akeno is pretty much there are a Mediator, I guess. During the last Episode they already had a meeting, where they probably talked about it that Issei will get the Sword. So that's why you can assume that it's not a trap or anything. Because if they would hurt Issei the whole point of the conference (aka the peace treaty) would be ruined. And like we knew from last Season, Rias wants someone to love her for who she is... as a person. Not because she is a Gremory and the Sister of one of the Satans etc. So here Rias said, even if Akeno is the Vice President, she is still Akeno and called Akeno. So she ask Issei how he sees Rias and he answered "You are President Rias" and Rias said "That's right. I am "President" but am I not "Rias" too?" with Issei answering "Yes! You are my Master and you're the High Class Devil Rias Gremory-sama!" Which basically is him acknowledging that she is more than just Rias, she is the Preisdent, she is his Master, she is a High Class Devil and a Gremory. When Rias only wanted to her that he says "You are Rias". Because of that Rias says that she is not the First, but the furthest away. Because Issei isn't as casual around her as with the others, he pretty much always uses honorifics. It's like she is on a pedestal while everyone else is on the ground, which makes her be further away. But we all know that Issei likes Rias more than anyone else, but it's just how she feels.
